Upperclass Foreign Citizen Financial Aid Application -- If you cannot download the form, let us know and we can mail one to you.
Parents' Income Tax Return or Wage Statement

Please send your parents' most recent tax return or official statement of yearly earnings.

Please circle figures for total income and total taxes and mark them accordingly.
Student's 2007 Federal Income Tax Return
(if filed)

If you filed a tax return for 2007, please send a copy of it.

If you did not file federal taxes for 2007, send this form to us instead.

Activity/Demographic Supplement Must be completed online by student --
Business/Farm Supplement
(if applicable)

Include appropriate corporate or partnership tax returns with all applicable schedules.
Noncustodial Parent Information
(if applicable)
In cases of separation or divorce, the noncustodial parent must complete a separate copy of the Upperclass Foreign Citizen Financial Aid Application and submit this to our office with a copy of his or her most recent tax return or wage statement.

Description
Letter of Special Circumstances
If the information we have required does not accurately reflect your family's financial situation, or if you believe a particular circumstance should receive special attention, we suggest that you write a separate letter and enclose it with the supporting documents that you submit with your application.
Leave of Absence
If you will be taking or returning from a leave of absence in Fall 2008 or Spring 2009, please inform us of your plans as soon as you can.
Study Abroad Supplement
If you are planning to Study Abroad in 2008-09, please submit the Study Abroad Supplement directly to our office.
   
IMPORTANT NOTICES
Late Fees
As of July 1, 2008 a $150 late application fee will be charged to his/her termbill account.
Eligibility
Students who complete applications after September 1, 2008 are, under normal circumstances, ineligible to receive financial aid for the fall term. If you have extenuating circumstances which will delay the completion of your financial aid application, contact your class officer as soon as possible.
